This Page is specifically designed to be used ONLY by Landlords who have paid the cost of eviction. This form is a tool to provide vital details about your case or schedule the physical remove of the tenant's belongings from your property.

After completing the form below, you will need to fax or email us a copy of the following four items immediately:

1. A copy of your Lease (call us if you don't have a lease with the tenant)
For those who have already filed the eviction, please send:
2. Your writ of possession & other pertinent court documents (signed by the Judge),
3. A copy of your receipt or confirmation for the sheriff's booking showing date (showing date and time of scheduled eviction) and
4. Proof of ownership (warranty deed or copy of most recent tax bill)
